Patient demographics, insurance information, relevant wound history or imaging, and your working diagnosis with ICD-10 if known. Fax to (541) 555-0100 or use the referral form on our For Physicians page. We handle benefits verification and prior authorization — typically within two business days.
Billing & codes
HBOT is billed under G0277 (per 30-minute interval) with facility code C1300. Your office bills nothing and loses nothing — the patient remains yours.
